Welcome to Heritage Family Medicine. We are a state of the art Family Practice Facility located in Madison, Alabama. Our providers include Dawn Mancuso, MD, Cindy McAdams, DO, Ashley Burchfield, MD, and Shelley Whitney, CRNP. We have a group of experienced RN’s and LPN’s who work with each Provider as a team to assist you and answer any questions.
Providers at Heritage Family Medicine see patients from newborns to the elderly. We provide on-site labs services via Lab Corp and X-rays for your convenience.
Office Hours are 8:00 a.m. to 4:30 p.m. Monday thru Friday, with lunch from 12:00 p.m. to 1:00 p.m. We are closed on all major holidays.
We are presently accepting new patients to our practice. In order for us to expedite your visit when arriving to our facility for the first time, please download our New Patient Forms and have them completed when you arrive for your appointment. Patients that do not have their paperwork filled out prior to their first appointment should come thirty minutes early to complete their paperwork prior to appointment time.
After office hours a Provider will be on call. Please note our Providers do not call in routine prescriptions or Narcotics after hours. If you have a Medical Emergency, please dial 911 or go to your nearest Emergency Room for treatment. Most insurances accepted. Please note that we are not a Medicaid provider.
